Your specific HSA Program may be referred to or branded as Anthem Blue Cross Blue Shield, Anthem Blue Cross, Empire Blue Cross Blue Shield or Empire Blue Cross. Please refer to your Welcome Materials for more information. References to your Administrator throughout this HSA Program experience will collectively mean Anthem.
As part of the Health Savings Account Program established by Anthem, your Administrator, you are being provided with the HSA Terms and Conditions that govern the use of your HSA. WealthCare Saver is the Custodian of your HSA. Please open and read through each document carefully. You should print and keep a copy of these documents for your records.
